**NEW Feedback Survey instructions are for all courses starting September 1, 2023 or after

Log in to SurveyMonkey - https://www.surveymonkey.com/user/sign-in/

All program associates have access to create and manage end of course surveys. If you cannot log in using your RRU username/password, ensure your manager has authorized your use of SurveyMonkey and open a service request ticket with Computer Services.


Be sure to log in with SSO (Single Sign On):

Click on "CREATE SURVEY"

Select “Start from Template”

Click the “TEAM TEMPLATES” tab


Select the End-of-Course Feedback Survey only - do not use the other templates

Select “COPY THIS SURVEY” in the bottom right corner

**Be sure to only use the name of your office’s Dean at the end of the intro text. REMOVE THE OTHER NAMES BEFORE SENDING.

Alternative method

Once you’ve used the template survey for your course, you may create a copy of that survey and use it like the template. To do this:

1. Login to SurveyMonkey

2. Click on My Surveys

3. Find the survey you wish to copy, click on the ‘…’ in the far right column, and select Make a copy

4. You can then edit this copy as needed, and create your collector

  • MULTIPLE INSTRUCTORS - Rate All by Cohort - This is for a course with multiple instructors where the respondent must select their cohort
  • MULTIPLE INSTRUCTORS - Rate All - This is for a course with multiple instructors
  • MULTIPLE INSTRUCTORS - Rate One - This is for a course with multiple instructors, but each respondent would have only been taught by one
